Job Description
The Community Health Supervisor (CHS) is responsible for overseeing and supporting community health activities within the assigned project area. The CHS ensures effective implementation of health promotion, disease prevention, and outreach services through the supervision of Community Health Workers (CHWs). The role includes capacity building, monitoring of community-based interventions, data collection and reporting, and coordination with health facilities and stakeholders to improve community access to essential health and nutrition services.
Responsibilities:
· The Community Health Supervisor is overall responsible for planning, executing, and reporting all CBHC activities as per project scope of work and Ministry of Public Health policies while maintaining close working coordination with local communities and health facilities.
· Assist HF in-charge in establishing community health shuras at the HF and health post levels with active participation from local community elders.
· Conduct regular health post visits for CHWs and attend local coordination and monthly health committee meetings at HF and HP levels.
· Organize and facilitate CHWs training and conduct supportive supervision from all HP within the HF catchment area, focusing on on-the-job training, receiving feedback from local communities/clients, and reporting to HF.
· Prepare a schedule for monthly health Shura meetings and ensure full and active participation of community elders.
· Support health facilities in organizing services in terms of client/patient flow control and screening.
· Maintain regular communication and establish close working coordination between the community and health facilities.
· Support the community in identifying and solving health problems.
· Assist HF staff in planning community health programs in the health facility and catchment areas.
· Conduct community awareness sessions as per the Health Facility plan and IEC materials provided by the Health Facility In-charge/doctor on a regular basis.
· Conduct regular health education sessions according to the health facility's health education plan and organize public awareness sessions in target communities and at the HP level with active involvement of CHWs.
· Regularly report to the health facility regarding all CBHC activities and CHWs' monthly performance.
· Use and report Health Committee information to monitor the health status and deliver the report to the head of the health facility.
Key Relations:
· Reports to health facility In-Charge and field supervisor.
· Coordinates with members of the basic health centre (BHC) and the Provision of Essential Life-Saving Healthcare Services to the catchment area population as per CBHC policy and project scope of work.
Key Results Expected:
· Coordination established between the health facility and the community in the surrounding areas.
· Improved local community awareness regarding the healthcare services available at the health facility.
· Increased community-level referrals to the health facility.
· Health promoted in the target areas of the health facility.
Safeguarding Responsibilities:

· Must read the Safeguarding Policy and either sign the Safeguarding Code of Conduct or sign a Code of Conduct that is consistent with or references this policy and Safeguarding Code of Conduct.

Job Requirements:
Education:
· Graduated from at least 12th grade; higher education, especially from a medical institute, will be preferred.
Work Experience:
· The candidate must have at least one year of previous experience in the relevant field or a similar working context.
Language Skills:
· Fluency in spoken and written Dari and Pashto is a requirement. English language skills are a plus.
Previous Working Experience:
· At least one year of experience in communities mainly involved in health education, awareness, community mobilization, and other social work.
